How deep should the soil be? And, what combination soil mix should I start out with for a first time raised bed? (Soil, manure, peat, compost, etc.)

As long as the beds are over earth the depth can be as little as a few inches. I personally prefer the beds to be 8" to 16" tall, as the extra height makes weeding much easier on my back.
